Now you will be able to produce your Annual Reports in a simplified manner using Data Fill-in Reports on your computer.

I have compiled the Annual Reports into PDF data entry files. To make use of these reports, you should obtain a copy of Adobe Acrobat Reader - version 5.0 or newer. This program is available from Adobe's Website - www.adobe.com and is a Free program for personal use.

If you visit the Adobe site, scroll down the screen until you see the Get Acrobat Reader Button, click on it and it will take you to another page where you can select the version required for your operating system, be it MAC or Windows version.

*Note: Some of these forms are specific to the North Central Region. However, if your region uses a similar form, I would be happy to generate a generic style form that allows you to enter your Region information. Also, if your region may use a completely different format I would be happy to generate a form for your specific region that allows you to input the data. Drop me an email and I will provide a mailing address where you could send me a blank copy of any of your Regional forms. I would setup separate pages on this site for each region's reports.
